Create New Custom Form

  1. Move the pointer over Configure.

  2. In Forms & Applications, click Custom Forms. 

  3. Click New for the form type you want to create. For example, to create an interview survey, click New Interview Survey. 

  4. Enter a Form Name. 

  5. Click Save.
