Observation Template Form Fields

When adding fields on the Details or Fields tabs of an observation template, you have the following options.

Text

• Presents user with a single-line text field for brief responses.
  

  

• No additional field configuration.

Text Area

• Presents user with a multi-line text box for lengthy responses.
  

  

• Text box provides options to expand the response field and open an advanced text editor with formatting options.
• No additional field configuration.

Description Text

• Presents user with descriptive or instructional text.
  

  

• Used to clarify/distinguish a portion of a form; no user response.
• No additional field configuration.

Date

• Presents user with a single-line text field; when clicked, a calendar displays.
  

  

• Only accepts a response in date format.
• No additional field configuration.

Boolean

• Presents user with on/off checkbox.
  

  

• Ensure field label is descriptive of what is being selected/deselected.
• No additional field configuration.

Single Option Drop Down

• Provides user with a drop-down menu allowing only one selection.
  

  

• Additional field configuration: choices must be entered and ordered.
  

  

Single Option Radio Button

• Provides user with a radio button menu allowing only one selection.
  

  

• Additional field configuration: choices must be entered and ordered.
  

  

Multiple Option Checkbox

• Provides user with a checkbox menu allowing multiple selections.
  

  

• Additional field configuration: choices must be entered and ordered.
  

  

File Set

• Provides user with the ability to attach a file to the form.
  

  

• No additional field configuration.
